Life & Accidental Death and Dismemberment Insurance– Benefit Amount: $30,000
– Coverage Reduction: 50% at age 65
– Termination: at age 71, or earlier retirement
Dependent Life– Benefit Amount Spouse: $5,000
– Benefit Amount Child(ren): $2,500
– Termination: at employee’s age 71, or earlier retirement
Critical Illness – Benefit Amount: $10,000
– Termination: at age 65, or earlier retirement
Extended health care– Annual Deductible: Nil
– Co-insurance for all eligible health expenses: 100% (unless otherwise indicated)
– Private Duty Nursing: $10,000 / calendar year, maximum 12 months, per condition
– Hearing Aids: $700 / 5 years
– Orthopedic Shoes and Orthotics: $300 / 12 months
– Hospital : Semi-Private Room
– Out-of-Canada Emergency: Unlimited maximum;  60-day trip duration
– Survivor Benefits: 24 months
– Termination: at retirement
Prescription drugsCo-insurance: 100%, pay-direct, enhanced generic
Paramedical– Co-insurance: 100%, $750 per calendar year per category of practitioner
– Included: Chiropractors, Physiotherapists, Psychologists/Social Workers, Dieticians, Podiatrists, Speech Therapists, Massage Therapists, Acupuncturists, Naturopaths & Osteopaths
Vision care– Eye Exams:  Co-insurance 100%, once per 24 months (every 12 months for children age 18 and under)
– Vision:  Co-insurance 100%, $300 per 24 months
Dental– Annual Deductible: Nil
– Basic: 80% to a maximum of $2,000 per calendar year (combined with Major)
– Major: 80% to a maximum of $2,000 per calendar year (combined with Basic)
– Dental Accident: 100%, Unlimited maximum
– Recall Frequency: once per 6 months
– Periodontal Scaling & Root Planing (combined): 14 units every 12 months
– Survivor benefits: 24 months
– Coverage terminates at retirement
Teladoc– Diagnostic and Treatment Support Services
– Access to specialists, resources, information and clinical guidance for serious physical or mental
Consult+– Virtual Health and Wellness Clinic
– Access to health care professionals, get prescription refills, referrals, assistance for non-urgent conditions, etc.
Employee & Family Assistance Program– Through HumanaCare
– Offers support with mental, financial, physical and emotional well-being
Eligibility & Waiting Period– Permanent  employees working at least 24 hours / week 
– Permanent, non-seasonal employee working at least 24 hours per week
